Registration Procedure. Please carefully review the table below,
and READ the appropriate registration guide. Each of the
documents listed must be completed and presented at registration, along with
the appropriate fees.
NOTE: ALL PLAYERS MUST SUBMIT A 1" wide by 1.5" high standard
headshot photograph.

1.
Print this form.
Insure the required wet-ink signature is completed.
The form MUST be signed by the Parent/Legal Guardian OR a Player age
18 or over.
Players who are age 18 or over MUST sign their own form. The wet ink
signature should be in black or blue. One copy of this form must be turned
in with the appropriate fees. Send

Uniforms.
These fees do not include the
cost of the Arden Park soccer uniform, cleats, balls, or shin guards. If additional
items needed, please email Lori Pedroncelli at
loriped@surewest.net with your order.

3. This year it is mandatory that
each family signs up for one or more volunteer tasks.
Fees.
The fee for
each player registering is $160. Registration forms may be submitted to the Arden Park Registrar.

Team Formation Process will begin in
June 2009. Registrations received after June 1st, 2009 may not have the
team and/or friend preferences incorporated into the placement of the player
on a team.
