Registration Procedure. Please carefully review the table below,
and READ the appropriate registration guide. Each of the
documents listed must be completed and presented at registration, along with
the appropriate fees.
NOTE: ALL PLAYERS
MUST SUBMIT PHOTOGRAPHS.

NOTES:
1.
Print this form.
Insure the required wet-ink signature is completed.
The form MUST be signed by the Parent/Legal Guardian OR a Player age
18 or over.
Players who are age 18 or over MUST sign their own form. The wet ink
signature should be in black or blue. Two copies of this form must be turned
in during soccer registration
with the appropriate fees. You may make a copy of the signed form and turn
the original and the copy in, or you may turn in two signed copies. Send

Uniforms.
These fees do not include the
cost of soccer cleats, balls, or shin guards.
The new Arden Park
Soccer Club
uniforms will be included in the
registration fee this year and includes a jersey, shorts, and socks.

3. This year it is mandatory that
each family signs up for one or more volunteer tasks.
Fees.
The current fee for
player registering is $160 per player. Registration forms may be submitted to the Arden Park Registrar.

Team Formation Process will begin in
June 2008. Registrations received after May 31, 2008 may not have the
team and/or friend preferences incorporated into the placement of the player
on a team.
